COUNSELING REFERRAL FORM:
Please fill out this form if you would like your student to see the school counselor for INDIVIDUAL or GROUP counseling or need assistance with an outside referral. Once I receive the submission, I will reach out to discuss your child's specific needs.
Although school counselors have extensive knowledge in child development, we are not mental health therapists and are legally and ethically limited in the type of support we can provide at school. If your student needs comprehensive or long term counseling (more than 8 sessions), I will be happy to assist you in finding outside resources or mental health supports.
